*Please note that all forms can be completed by a parent/guardian, except for the Application for Participation,
which needs to be completed be a physician.

What Do These Forms Mean?

    Application for Participation
    This is the medical form for the Athlete to participate and MUST be signed by a licensed physician, licensed physician’s
    assistant, or licensed nurse practitioner. No other medical forms may be accepted.

    Athlete Sports Interest Form
    This form will allow us to notify the Athlete when a particular sports season that interests them begins.

    Athlete Release Form
    Self explanatory- must be signed by a parent or guardian if the athlete is a minor.

    Special Provisions Regarding Medical Treatment
    Form is to be filled out if you DO NOT want your athlete to receive medical treatment die to religious beliefs. If you are ok
    with treatment, please disregard this form and discard it. It does not have to be submitted.

    Athlete Code of Conduct
    In most cases, a parent or guardian will read this form to the athlete and in either case, a signature is require by the
    athlete and if explained by a parent or guardian, their signature is required as well.

    Family Member Code of Conduct
    Relatively new form and we are asking you to please read and sign it accordingly.

    Atlanto-Axial Instability Status Form
    For athletes with Down Syndrome and is only required to be filled out on the initial application process in Special
    Olympics. Once again a licensed physician, licensed physician’s assistant, or licensed nurse practitioner MUST sign this
    form.

    Atlanto-Axial Instability Release Form
    Also for athletes with Down Syndrome and is only required to be filled out on the initial application process. The
    signature process is the same as above.
